Executive Clinical Snapshot
Prof. Jianguo Xu, MD, PhD, is Director and Chief Physician of the Department of Neurosurgery at West China Hospital, Sichuan University. Renowned across China and internationally as a premier master microneurosurgeon, Prof. Xu has devoted over three decades to the most perilous frontiers of the human brain: Complex Cerebrovascular Malformations, Deep-Seated Cerebral Arteriovenous Malformations (AVMs), Giant Intracranial Aneurysms, Brainstem Lesions, and Advanced Skull Base Neoplasms. Combining ultra-delicate microscopic dissection with intraoperative real-time indocyanine green (ICG) angiography, continuous multimodality electrophysiological monitoring, and hybrid endovascular capabilities, Prof. Xu consistently achieves complete lesion eradication while zealously safeguarding eloquent neurological networks and vital cranial nerve functions.

Microsurgical Mastery: Navigating Eloquent Brain Territories
Cerebral vascular lesions located within eloquent motor, sensory, or speech cortices—as well as deep in the brainstem and petroclival skull base—present astronomical surgical mortality and morbidity risks when approached with conventional neurosurgical techniques. Prof. Xu has pioneered and refined several key microneurosurgical methodologies:
Key Surgical Breakthroughs Spearheaded by Prof. Jianguo Xu:
- Subcortical Pathway & Sulcal Dissection for Deep AVMs: Utilizing high-definition fiber tractography (DTI) and functional MRI guidance, Prof. Xu navigates deep natural sulcal corridors to reach deep-seated AVM niduses without traversing vital corticospinal or arcuate tract fibers.
- Hybrid Surgical & Angiographic Suite Coordination: Executes complex surgeries inside integrated hybrid operating rooms, combining microscopic clipping/resection with immediate on-table digital subtraction angiography (DSA) and micro-Doppler flowmetry to confirm complete obliteration and normal vessel patency.
- Cranial Nerve Preservation in Petroclival & Acoustic Skull Base Surgery: Employs continuous facial (CN VII), trigeminal (CN V), and lower cranial nerve (CN IX-XI) electrophysiological monitoring coupled with ultrasonic bone scalpel osteotomies, achieving anatomical preservation of the facial nerve in over 95% of giant vestibular schwannomas.
- Direct Revascularization for Moyamoya Disease & Complex Aneurysms: Masters high-flow and low-flow intracranial-to-extracranial (IC-EC) bypass techniques (STA-MCA anastomoses) with vessels as fine as 0.8 mm, restoring robust collateral cerebral perfusion.
Comprehensive Surgical Spectrum & Clinical Scope
- Brain Arteriovenous Malformations (AVMs): Spetzler-Martin Grade III to V cerebral and cerebellar AVMs, dural arteriovenous fistulas (dAVFs), and Galenic vein malformations.
- Complex & Giant Intracranial Aneurysms: Paraclinoid, basilar apex, posterior inferior cerebellar artery (PICA), and middle cerebral artery bifurcated aneurysms requiring microscopic clipping, trapping, or bypass reconstruction.
- Cerebral & Brainstem Cavernous Malformations: Intrinsic pontine, midbrain, and medullary cavernomas causing recurrent hemorrhagic stroke.
- Complex Skull Base Tumors: Petroclival meningiomas, large acoustic neuromas (vestibular schwannomas), trigeminal schwannomas, jugular foramen tumors, and chordomas.
- Revascularization & Moyamoya Disease: Combined direct (STA-MCA) and indirect (EDAS) bypass surgeries for adult and pediatric Moyamoya disease.
Benchmark Clinical Scenarios
Scenario A: Spetzler-Martin Grade IV Thalamic AVM in an Eloquent Speech Area
A 34-year-old university lecturer suffered a sudden intraventricular hemorrhage secondary to a 3.5 cm high-flow AVM situated in the dominant left thalamus and posterior limb of the internal capsule. Other centers deemed the lesion inoperable due to imminent hemiplegia and aphasia risks. Prof. Xu conducted a transcallosal-interforniceal approach guided by neuronavigation and tractography. Under intense continuous motor-evoked potential monitoring, the feeding lenticulostriate and choroidal arteries were sequentially disconnected and the nidus was excised en bloc. Postoperative DSA verified 100% cure; the patient experienced zero motor or speech deficits and resumed lecturing within 4 months.
Scenario B: Giant Petroclival Meningioma Compressing the Brainstem
A 48-year-old woman presented with severe gait ataxia, diplopia, and progressive dysphagia caused by a 4.2 cm petroclival meningioma encasing the basilar artery and severely displacing the pons. Prof. Xu performed an extended retrosigmoid-transtentorial approach. Through microscopic micro-dissection over 9 hours, Simpson Grade II total tumor resection was achieved with complete preservation of the basilar artery and cranial nerves III through VIII. At 18-month follow-up, cranial MRI confirmed no recurrence and all neurological deficits resolved.
Scientific Publications & Neurosurgical Education
Prof. Jianguo Xu has published over 160 papers in leading neurosurgical journals including Journal of Neurosurgery, Neurosurgery, World Neurosurgery, and Acta Neurochirurgica. As one of China's foremost surgical trainers, he has trained generations of neurosurgical fellows and regularly leads masterclass surgical demonstrations in complex cerebrovascular anatomy and skull base microsurgery.
