How to Use a Moxa Box at Home Safely

What is a Moxa Box?

A moxa box (艾灸盒, ài jiǔ hé) is one of the most practical and beginner-friendly tools in Traditional Chinese Medicine. It is a wooden or metal box designed to hold burning moxa sticks or moxa cones, allowing sustained, even heat to penetrate acupuncture points on the body — without requiring a practitioner to hold the moxa manually.

Moxa boxes are widely used in Shanghai's TCM hospitals and are the tool most commonly recommended by our doctors at China Medical Concierge Shanghai (CMCS) for patients who want to continue moxibustion therapy at home after returning to their countries.

Types of Moxa Boxes

  • Single-hole wooden moxa box: Compact, ideal for targeting specific points on the wrist, knee, or ankle
  • Double-hole wooden moxa box: Covers a wider area — great for the lower back or shoulders
  • Triple-hole wooden moxa box: Best for the abdomen or full lower back treatment
  • Metal moxa box with belt: Wearable design — allows hands-free treatment while sitting or lying down

What You Need Before You Start

  • A moxa box (wooden or metal)
  • Moxa sticks or moxa rolls (pure mugwort, aged 3+ years recommended)
  • A lighter or matches
  • A fireproof tray or ashtray
  • A timer
  • Good ventilation — open a window
  • A thin cloth or towel to place between the box and skin if needed

Step-by-Step: How to Use a Moxa Box

  1. Prepare your space: Open a window for ventilation. Lay down on a comfortable surface — a yoga mat or bed works well. Have your fireproof tray nearby.
  2. Light the moxa stick: Hold the moxa stick over a flame for 15–20 seconds until the tip glows red and begins to smoke. Blow gently to encourage even burning.
  3. Insert the moxa into the box: Place the lit end of the moxa stick into the hole of the moxa box, securing it at the appropriate height (usually 2–3 cm above the mesh screen).
  4. Position the box on your body: Place the moxa box directly on the target area — most commonly the abdomen (CV12, CV6), lower back (BL23, GV4), or knees (ST36). The mesh screen prevents direct contact with the skin.
  5. Adjust for comfort: You should feel a pleasant, penetrating warmth — not burning or stinging. If it feels too hot, place a thin cloth between the box and your skin, or raise the moxa stick slightly higher in the box.
  6. Treat for 15–30 minutes: Most TCM practitioners recommend 20 minutes per session for general wellness. For specific conditions, your doctor may advise longer sessions.
  7. Extinguish safely: When finished, remove the moxa stick and press the lit end firmly into sand or a fireproof container to extinguish completely. Never leave burning moxa unattended.

Which Acupuncture Points to Target

The most commonly recommended points for home moxa box use:

  • CV6 (Qihai — Sea of Qi): 1.5 inches below the navel — for energy, digestion, and immune support
  • CV12 (Zhongwan): 4 inches above the navel — for digestive health and appetite
  • ST36 (Zusanli): Below the knee — for immunity, energy, and overall wellness
  • BL23 (Shenshu): Lower back, beside the spine — for kidney energy, back pain, and fatigue
  • GV4 (Mingmen): Centre of the lower back — for vitality and cold conditions

Note: Always consult your TCM practitioner or CMCS medical team for personalized point recommendations based on your specific condition.

Safety Guidelines

  • ✅ Always use in a well-ventilated room
  • ✅ Never fall asleep during a session
  • ✅ Keep water or a damp cloth nearby
  • ✅ Check skin regularly — mild redness is normal, blistering is not
  • ❌ Do not use on broken, inflamed, or sunburned skin
  • ❌ Do not use during fever or acute infection
  • ❌ Avoid use during pregnancy without medical guidance
  • ❌ Do not use near the face or on sensitive areas

How Often Should You Use a Moxa Box?

For general wellness maintenance, most TCM practitioners recommend:

  • Beginners: 2–3 times per week, 15–20 minutes per session
  • Recovery support: Daily sessions of 20–30 minutes (as directed by your doctor)
  • Seasonal wellness: Intensive sessions at the change of seasons (especially autumn and winter)
