Fever in Children: When to Worry and When to Wait

Introduction

Fever is one of the most common reasons parents rush to the emergency room — but not every fever requires urgent medical attention. Understanding what's normal, what's concerning, and when to act can save you stress and help your child recover faster.

What Is a Fever?

A fever is generally defined as a body temperature above 38°C (100.4°F). It is the body's natural immune response to infection and is usually a sign that the immune system is working.

  • Low-grade fever: 38°C – 38.9°C
  • Moderate fever: 39°C – 39.9°C
  • High fever: 40°C and above

Managing Fever by Age Group

Newborns (0–3 months)

Any fever in a newborn is a medical emergency. Go to the hospital immediately — do not wait.

Infants (3–12 months)

A fever above 39°C lasting more than 24 hours warrants a doctor's visit. Watch for signs of dehydration (dry mouth, no tears, reduced urination).

Toddlers & Older Children (1–12 years)

Most fevers in this age group are caused by viral infections and resolve within 3–5 days. Use age-appropriate fever reducers (paracetamol or ibuprofen) and ensure adequate hydration.

Red Flag Symptoms — Seek Immediate Care

  • Fever in a baby under 3 months
  • Fever above 40°C that doesn't respond to medication
  • Seizures or convulsions
  • Difficulty breathing or persistent cough
  • Rash that doesn't fade when pressed
  • Extreme lethargy or unresponsiveness
  • Stiff neck or sensitivity to light
  • Fever lasting more than 5 days

Home Care Tips

  • Keep the child hydrated with water, diluted juice, or oral rehydration solution
  • Dress in light clothing and keep the room cool
  • Use a digital thermometer for accurate readings (rectal for infants)
  • Do NOT use aspirin in children under 16 (risk of Reye's syndrome)
