Dear Doctor / International Patient Department,
My name is Saja, and I am a patient from Kuwait. I am considering traveling to Thailand for complex facial revision surgery and would like to consult with a highly experienced plastic, reconstructive, and facial surgeon.
I had permanent facial filler/material injected in my face in 2007. Since the material has been present for many years, I am concerned that it may have caused fibrosis, scar tissue, migration, or inflammatory changes in the surrounding tissues.
My goal is NOT simply to have a facelift.
I would like a specialist with significant experience in complex revision facial surgery, particularly:
• Removal of old permanent facial filler/material and associated fibrosis or granuloma, when medically and surgically possible
My preference regarding the surgery• Revision surgery after previous permanent facial injections
• Careful identification and preservation of the facial nerve and its branches
• Reconstruction or restoration of the facial tissues after removal of the material, if necessary
• Facelift using the most appropriate technique, such as Deep Plane or SMAS, depending on my anatomy and previous injections
• Neck lift and improvement of the jawline and neck contour
• A natural-looking result while preserving normal facial movement and expression
Because I am traveling internationally from Kuwait, it would be very difficult for me to return to Thailand for a second operation.
Therefore, my strong preference is, if medically safe and appropriate, to have the following performed during ONE surgical procedure:
1. Removal of the permanent facial material and associated fibrosis/scar tissue, as safely as possible
2. Any necessary reconstruction or restoration of the facial tissues
3. Facelift
4. Neck lift
5. Improvement of the jawline and neck contour
I completely understand that patient safety comes first and that the surgeon may determine that some procedures should not be combined. However, if it is medically safe, I would strongly prefer to complete the treatment in one surgical stage.
I understand that complete removal of permanent material may not always be possible or safe, particularly if it is deeply integrated into the tissues or located close to the facial nerve. My priority is to protect the facial nerve and avoid unnecessary damage rather than attempting to remove every particle at any cost.
Before deciding on surgery, I would therefore like the surgeon to evaluate:
• The location and depth of the permanent material
• The extent of fibrosis or scar tissue
• Whether the material has migrated
• Whether it is close to or involving any branches of the facial nerve
• Whether ultrasound, MRI, CT, or another imaging study is recommended
• Whether removal and facelift/neck lift can safely be performed during the same operation
• Which facelift and neck-lift technique would be most appropriate for my anatomy
• What reconstructive techniques, if any, may be needed after removing the material
I can provide clear photographs from the front, both profiles, and 45-degree views, as well as any previous medical reports or information regarding the material injected in 2007.
I would be grateful if you could refer my case specifically to the surgeon at your hospital who has the greatest experience with permanent filler removal, complex revision facial surgery, facial nerve preservation, facelift, and neck lift, rather than simply a surgeon who performs routine facelifts.
